What is the rock drug (crack cocaine)?

The rock drug is crack cocaine: cocaine turned into small off-white rocks that are smoked, giving a high that lasts only 5 to 10 minutes.

What is the rock drug?

Rock is a street name for crack cocaine, a smokable form of cocaine sold as hard, off-white lumps that look like small crystals; some people simply say cocaine rock. The name crack comes from the crackling sound the rocks make when heated. Elsewhere it is also called candy, nuggets or grit.

Crack is a stimulant drug. Smoked, it reaches the brain within seconds, which is why the high is so strong and so short: NIDA says the high from smoking “is more immediate but may last only 5 to 10 minutes”. Energy, confidence and a racing mind come first, then a crash that leaves the person wanting the next rock straight away.

Why does crack create a cycle of dependence so fast?

Crack creates dependence quickly because each high is short and the low after it is sharp: “the faster the drug is absorbed, the more intense the resulting high, but also the shorter its duration”, in NIDA’s words. One rock becomes a night of rocks, and the night ends when the money or the supply runs out. Over weeks the person needs crack just to feel normal, and anything that interrupts the supply brings exhaustion, low mood and craving.

How does the rock drug differ from cocaine?

Rock and powder cocaine are the same drug in two forms. Powder is usually snorted; rock is smoked. The difference is speed and price:

Rock (crack) and powder cocaine compared
Rock (crack)Powder cocaine
How it is takenSmoked from a pipeSnorted, sometimes injected
How fast it actsAlmost at once, more intenseSlower, less intense
How long the high lastsAbout 5 to 10 minutesAbout 15 to 30 minutes when snorted
How it is soldCheap single rocksUsually in larger, dearer amounts

Cheap single rocks make crack easy to start and hard to stop, because the next one is always affordable. What else is in a street rock is never known to the person smoking it. Our page on cocaine addiction signs covers the powder.

What are the long-term effects of crack cocaine?

  • Lungs: “smoking crack cocaine damages the lungs and can worsen asthma” (NIDA); a cough, chest pain and shortness of breath are common complaints.
  • Heart: the MSD Manual lists chest pain from reduced blood flow to the heart, spasm of the coronary arteries and dangerous heart rhythms among cocaine’s effects.
  • Body: no appetite, weight loss and broken sleep.
  • Mind: anxiety, paranoia and, with heavy use, psychosis; depression between runs.
  • Mixing: crack with alcohol or heroin is more dangerous than either alone and can be fatal. Our page on cocaine and alcohol explains why.

Chest pain, a fit, sudden confusion or collapse after crack is an emergency: call 10177, or 112 from a cellphone, and say what was taken.

Crack in South Africa

Crack matters most in KwaZulu-Natal, where crack and cocaine were the main drug for 10% of people entering treatment in July to December 2025, against 4% in Gauteng and 3% in the Western Cape, according to SACENDU. Crack is illegal: cocaine and any preparation of the coca leaf are listed in Schedule 2 of the Drugs and Drug Trafficking Act 140 of 1992, and section 4 says no person shall use or have it in their possession.

When is it time to get help for crack?

It is time when the use continues despite what it costs. Look for:

  • using again and again despite health, money or family problems;
  • cravings strong enough that the person cannot control how often they use;
  • work, studies or children being neglected;
  • secrecy, suspicion, sudden agitation, money and belongings going missing;
  • earlier attempts to stop that did not hold.

Stopping crack brings a crash: the MSD Manual describes “somnolence, difficulty concentrating, increased appetite, and depression” after heavy use. The danger is the low mood. If there are thoughts of suicide, call the SADAG Suicide Crisis Helpline on 0800 567 567. If crack is the only drug, a week without it is a fair test; if alcohol or pills are used to come down, do not stop those suddenly on your own and message us first.

How is crack addiction treated?

Crack runs on triggers, so the first month teaches the person to spot them and the second month works on what sits under them. There is no medicine for crack, so treatment is therapy in a safe place, away from the supply, with the work aimed at why the person needed it. At our house in Lakeside, close to Muizenberg beach, that runs as one plan:

  • The first month, living with us: drug and alcohol tests on arrival and at random, a medical assessment by our GP, Dr John Wulz, one-to-one therapy typically three times a week, groups and a 12-step meeting every evening.
  • The second month goes after the reason: CBT and DBT for craving and urges, schema therapy for old patterns, EMDR for trauma. Ruby Weber, our psychologist, has more than 20 years in the addictions field and leads the dual diagnosis work.

For a family in Durban, Cape Town is a short direct flight, and it puts distance between the person and the people who sell. Admission can be immediate and typically happens within three days of first contact.
