It takes adults aged 18 and older without a social worker’s referral. A free or subsidised bed in a residential centre comes through a social worker’s assessment and a waiting list, and the whole province had 671 funded inpatient bed spaces in 2023/24. Renewed Life Center is a private residential centre in Lakeside, for families who cannot wait that long.
Which national helplines refer you to free rehab?
Three free lines cover the whole country and can point you to a social worker or a treatment service near you; the Western Cape adds a line of its own. SADAG lists them, and the substance abuse line belongs to the Department of Social Development.
Substance abuse helpline (DSD, listed by SADAG)
- Phone: 0800 12 13 14, free, 24 hours
- SMS: 32312
- What it does: confidential counselling and a referral to the treatment service that covers your area
Ke Moja WhatsApp line
- WhatsApp: 087 163 2025, 08:00 to 17:00
- What it does: counselling by message and directions to local treatment services
Western Cape DSD line
- Phone: 0800 220 250, free
- What it does: tells you which local DSD office covers your suburb, which is where the referral for a funded bed starts
Suicide crisis line
- Phone: 0800 567 567, free, 24 hours, for anyone who talks about ending their life
Free support groups: AA and NA
Alcoholics Anonymous and Narcotics Anonymous meet free of charge in every province, in person and online. The AA helpline is 0861 435 722, open 24 hours, and the NA phoneline is 0861 00 6962. Our guide to Alcoholics Anonymous in South Africa explains how to find a meeting.

Free Matrix outpatient sites in Cape Town
Matrix is free. It is the City of Cape Town’s outpatient programme, 16 weeks long, with several intensive sessions a week, for people aged 18 and older. You can go straight to a Matrix clinic, and after a professional assessment you may be able to start right away. You sleep at home and keep your job. The drinking or using stays within reach, too.

| Scottsdene | Scottsdene Clinic, Eoan Street, Kraaifontein | 021 444 8777 |
All eight sites share one email address: matrix@capetown.gov.za. The Delft South page lists the Matrix programme hours as Monday to Friday, 08:00 to 16:30. The directory prints the same number for Tafelsig and Delft South; if nobody answers, call the DSD on 0800 220 250.
Matrix suits someone who still has a safe home to sleep in and can reach a clinic several times a week. It is a harder fit for a person who drinks heavily every day, because outpatient sessions do not cover the first days off alcohol, and that person should not stop suddenly on their own. Message us first: we will tell you whether a medical detox is needed and arrange it. More on how day programmes work: outpatient treatment for substance use.
Daily drinking or drug abuse is often how a person has been coping with something else, such as grief or a depression that never lifted. At Renewed Life Center the first month of residential care starts on that reason, with a psychologist and trauma therapy.

How do you get into a free or government rehab in Cape Town?
A free residential bed starts with an assessment and a social worker’s report; some centres, such as Toevlug, also accept the application from a doctor or a pastor. Matrix needs no social worker’s referral, only an assessment at the clinic. The provincial directory says the assessment can be done “at a local office of the Department of Social Development, or any community-based organisation with substance use disorder services”.
The route, step by step
-
Get assessed
Go to your local DSD office, a SANCA branch or another community service from the list above, and take your ID. The helplines tell you which office covers your area.
-
Hear which level of care fits
The assessment points to one of three tracks: early intervention, community-based treatment (regular sessions while you keep working and living at home), or inpatient treatment.
-
For a bed, the social worker applies
The social worker writes the report and sends the application to an inpatient centre. Toevlug also takes applications from a doctor or a pastor.
-
Wait for a place
Each centre keeps its own list. Matrix and SANCA do not need a bed to begin, so outpatient sessions can run while the application waits.
The regional DSD offices, as listed in the November 2025 directory: Metro North 021 483 7673, Metro East 021 360 4200, Metro South 021 763 6200, Cape Winelands 023 348 5300, West Coast 022 713 2272, and Eden Karoo 044 814 1687. The head office is at Union House, 14 Queen Victoria Street, in the city centre.
How long is the waiting list for a state rehab?
There is no official waiting time. Asked in Parliament for the average wait, the Minister of Social Development answered in October 2025: “There is no specific average waiting period as the cases differs from one another.” Lise van den Dool of the rehab group U-Turn told GroundUp that state-funded beds have “a long waiting period”, and that when a person is ready, “you’ve got a short window period”. The 671 funded bed spaces of 2023/24 covered the whole province.
While you wait, keep the social worker’s number on your phone and ask for news every week or two, and book a Matrix or SANCA appointment so that something starts now. Families can read how to talk to a loved one about rehab before the next hard conversation.
A person who said yes on a bad Sunday may say no three weeks later, once the hangover has faded and the house has gone quiet again. And the wait does nothing to the reason behind the drinking. That is the part we start on in the first month at Renewed Life Center. Tell us where you are on the list, and we will say how soon we can admit.

Gauteng has a state centre in Cullinan that reopened in 2026 with more beds, a free clinic at Sterkfontein Hospital and non-profit centres around Johannesburg and Pretoria. What if private rehab in Cape Town is an option?
A private residential stay works on what the drinking or using was covering, and it can start within days. Renewed Life Center is in Lakeside, close to Muizenberg beach.
Depression, anxiety and trauma are treated here alongside the addiction, in one house and on one plan, because NIDA advises treating these conditions at the same time rather than separately (NIDA). Ruby Weber, our psychologist, has more than 20 years in the addictions field with South African and international clients and leads the trauma and dual diagnosis work. A psychiatrist selects medication where it is needed. More on this: dual diagnosis support.
We use cognitive behavioural therapy, which NICE recommends for alcohol dependence, alongside DBT, family therapy, relapse prevention and daily 12-step work. On top of it, schema therapy works on old patterns. For trauma we use EMDR, which NICE and the WHO recommend for PTSD, together with TRE. NIDA’s Principles of Drug Addiction Treatment tie stays under 90 days to limited effectiveness. We plan in months because the work comes in stages: Phase 1 makes you safe and steady, Phase 2 goes deeper into trauma and relationships, and the Recovery Integration Programme or the Sober Living Programme takes you back into ordinary life step by step.
The people who run it know the work first-hand. Nikki Edwards, Programme Director and Clinical Manager, has 11 years in recovery. John Roberts, Senior Recovery Specialist and Operations Manager, has 39. Leila Bolland, social worker and therapist, has been in practice since 2015, and Dr John Wulz, our GP, sees every client.
The day starts at 07:00 with a morning group, then a one-to-one session, groups until four, step work and a 12-step meeting every evening. On Saturdays the house goes out for a hike, pottery or lunch in town. Drug and alcohol tests happen on arrival and at random, so everyone in the house stays safe. More on the stay: what happens in rehab, day by day.
Families usually write to us at a moment they recognise. The social worker has said “a few weeks”, and meanwhile he has started drinking before work. She has promised to stop three times since Easter. Families often start this themselves: in the first half of 2025, 40% of people who entered specialist treatment in the Western Cape came on their own or through family and friends, according to SACENDU, the SAMRC’s treatment monitor. You can write on someone else’s behalf, and the family group meets every Wednesday at 18:00 on Zoom.
Someone who drinks heavily every day, or takes benzos or sleeping tablets, should not stop suddenly at home while waiting for a bed. Message us first: we will tell you whether a medical detox is needed and arrange it. Detox runs on site with 24-hour nursing supervision; read more on medical detox in Cape Town. If someone has a seizure, is confused or sees things that are not there, call 10177 for an ambulance, or 112 from a mobile.
Admission can be immediate and typically happens within three days of first contact. You pay us directly, so no scheme decides how long you stay; the fee depends on the programme and its length, and we send it in writing the same day as the free first call. Our guide to what rehab costs in Cape Town shows how private fees are built. Families who cannot wait for a funded bed come to us for that: the reason behind the drinking treated from the first week, on cognitive behavioural therapy, which NICE recommends for alcohol dependence, with family work, relapse prevention and trauma therapy added, from people who have lived and worked in this field for decades. Tell us who the bed is for, and you will hear back the same day.
What else do people ask about free rehab in South Africa?
An assessment at a local DSD office or a community service such as SANCA, then, if a bed fits, a social worker’s report and application, then a place on the centre’s list. Matrix in Cape Town skips the social worker: you go to the clinic, get assessed, and can often start straight away.
Matrix costs nothing, and the government residential centres are funded by the state. Some subsidised non-profit centres ask for a contribution, such as Hesketh King’s R1,200, so ask before the admission date. What a free place costs most is time.
SANCA is a non-profit with branches across the country that run early intervention, outpatient treatment, aftercare and a few inpatient centres. Whether it is free depends on the branch and the programme, so ask on the first call.
Usually, for residential beds, and each centre keeps its own list; the Minister told Parliament in October 2025 that there is no average waiting period. Outpatient services start sooner.
Time and depth. A funded bed comes through an assessment, an application and a list. A private centre admits within days, and at Renewed Life Center the stay includes a GP who sees every client, therapy every weekday and a plan for depression, anxiety or trauma alongside the addiction. Private and public centres around the country are compared in rehab centres in South Africa.
It depends on the centre. Hesketh King asks for an application with a registered social worker’s report, Toevlug accepts a form from a social worker, doctor or pastor, and others ask you to call them first. The helpline on 0800 12 13 14 points you to the right office.
Partly. Substance use disorder is a prescribed minimum benefit, which the Council for Medical Schemes describes as hospital-based management of up to 3 weeks a year; some plans cover more. With us you pay directly, so no scheme cap decides when you go home.
Call 0800 12 13 14 or, in the Western Cape, 0800 220 250: both tell you which DSD office covers your area. In Cape Town, the nearest of the eight Matrix clinics is usually the quickest free start.
There are four: free outpatient programmes such as Matrix in Cape Town, SANCA early intervention and aftercare, state-funded residential centres reached through a social worker, and private residential rehab. The helpline on 0800 12 13 14 tells you which of these covers your area.
Start with the free and subsidised places listed above: Matrix, SANCA branches and the government centres cost little or nothing, but residential beds run on a social worker’s referral and a waiting list. Non-profit centres such as Hesketh King ask for a contribution, so ask the price before the admission date. A private centre costs more and admits within days.
Call the Department of Social Development substance abuse helpline on 0800 12 13 14 for a referral; for depression and anxiety, SADAG answers on 011 234 4837 from 8:00 to 20:00. Stikland Hospital in Bellville is a government psychiatric hospital, and the Matrix and SANCA teams can refer you on. If drinking or using sits alongside depression, say so on the first call.
A few. Ernest Malgas in Gqeberha is listed for ages 13 to 18, and Elim Clinic in Kempton Park runs a programme for under-18s. Kensington in Maitland no longer takes adults, but its places go to children in secure care, not to teenage rehab. Matrix is for adults aged 18 and older.
The DSD directory lists the Saartjie Baartman Centre for Women and Children in Athlone (021 633 5287), and Matrix sites and SANCA branches see women as outpatients. If she needs a bed and the list is long, message us.
These public services are built around local DSD offices and clinics for people who live here, so a family abroad usually looks at a private centre. International clients are met at Cape Town airport and taken back when they leave, we help with the visa paperwork, and the Wednesday family group at 18:00 Cape Town time runs on Zoom. From the US, plan the cost as self-pay. Message us with the country you are writing from.
For a referral to a free service, call 0800 12 13 14 at any hour or WhatsApp Ke Moja on 087 163 2025 in office hours. For a bed this week, message us. If someone is unconscious or fitting, call 10177 or 112 now.
