What happens to your body when you stop drinking?

Sleep, liver, blood pressure, weight and mood after you stop drinking: what has been measured, what has not, and when the first days need a doctor.

  • The first days come first. If your body is dependent on alcohol, stopping suddenly can be very dangerous (NHS), and withdrawal begins within 6 to 12 hours of the last drink. Message us before you stop and we tell you whether you need a medical detox.
  • The body then gives some things back, on different clocks. In one UK study of drinkers without dependence, the liver enzyme GGT fell by 28.6% in a month. Fatty liver can recover over months or years. The risk of mouth and oesophageal cancers falls over years.

Most people who type this question are standing at one of two doors. Some drink most evenings and wonder whether a dry month would be worth it. Others drink every day and want to know what the first week will do to them. The answer is different for each of them, and some of the popular claims have no evidence behind them at all.

What happens in the first days after your last drink?

If your body is dependent on alcohol, the first days bring withdrawal, not benefits. Symptoms begin within 6 to 12 hours of your last drink and usually last from 3 to 7 days (NHS), and stopping suddenly can be very dangerous in that case. Call for an ambulance now if, after stopping, someone is having a seizure; or confused, or seeing and hearing things that are not there. In Cape Town, the Western Cape Government lists 10177 for an ambulance and 112 from a mobile phone.

Will you sleep better after you stop?

Often, but not overnight, and not straight away if you were dependent. The NHS lists better mood, memory and quality of sleep among the benefits of cutting down. A review of people with alcohol use disorder found only limited recovery of sleep disturbances within the first 30 days of abstinence.

The same review says some people who stay abstinent for a long time still have persistent sleep problems. Poor sleep in the first weeks after stopping is common. If it drags on, tell us and it goes into the assessment.

What helps is dull and works: a fixed bedtime, a dark room, no screen for the last half hour, and no coffee after lunch. The evening itself, the hour the first drink used to arrive, is covered in the guide to stopping safely.

What happens to your liver, and can fatty liver recover?

Fatty liver can recover, but the timescale is months or years, not days. The NHS says the liver can usually recover if you stop drinking, and Liver UK says the damage from alcohol-related fatty liver may be reversed if you stop for a period of time (months or years).

Fatty liver is the early stage. Both sources speak about that stage. Anything more advanced is for the medical assessment with our resident GP.

You may hear about GGT, a liver enzyme measured in a routine blood test. In a UK study of moderate to heavy drinkers without dependence or liver disease, GGT fell by 28.6% after one month without alcohol. That is the result of one study in one kind of drinker. It shows the direction of travel and gives no date for anyone else. If you drink heavily, get a liver blood test before you stop and another a few months on, so you can see your own numbers.

Does your blood pressure come down?

If you were drinking a lot, yes. A review of 36 trials found that people who drank more than two drinks a day and cut back lowered their blood pressure, while for people drinking two or fewer a reduction was not associated with a significant fall.

The biggest effect in that review, published in The Lancet Public Health in 2017, was among people who drank six or more drinks a day and cut their intake by about half: systolic pressure fell by 5.50 mm Hg and diastolic by 3.97. That was cutting down, not stopping. In the UK study above, systolic pressure fell by 6.6% and diastolic by 6.3% after a month without alcohol.

The trials count a “drink” by their own measure, which is not the same everywhere, so count what you actually pour in grams, not glasses. And have your blood pressure checked before and after: you will know it has changed only if you measured it.

Will you lose weight, and how many calories does it save?

Probably some, because alcohol is calorie-dense. The NHS says a standard glass of wine can contain up to 158 calories, and some pints of stronger lager up to 222. Alcohol itself carries 7 calories per gram, almost as many as a gram of fat.

In the UK study, weight fell by a median of 1.5% after one month without alcohol. Some people swap the wine for sugary drinks or late-night food, and the scales do not move.

Will anxiety and low mood improve?

If you were dependent, anxiety and low mood often ease within weeks. A review of studies of people with alcohol use disorder found that clinical depression and anxiety symptoms dropped to a normal range after 2-4 weeks of abstinence. The same review cautions against diagnosing a separate depression or anxiety disorder in the first few weeks.

If your anxiety or low mood is no better after that stretch, it may be a condition of its own that the drinking was covering. That is treatable, and it is a different job from stopping drinking.

Does quitting lower your cancer risk, and what about immunity?

The risk of some cancers falls, slowly, over years. The US National Cancer Institute says stopping is associated with lower risks of oral cavity and oesophageal cancers and possibly of throat, breast and colorectal cancers, and that it may take years to reach the risk of people who never drank. The World Health Organization calls alcohol an established carcinogen.

For immunity there is no timetable. The US National Institute on Alcohol Abuse and Alcoholism (NIAAA) says drinking too much can weaken the immune system. It gives no date for recovery, so a claim that your immune system is back to normal after 30 days has nothing behind it.

What changes over the weeks, months and years, and what has no evidence?

Measured changes run from hours to years: withdrawal starts within hours, mood and liver blood tests shift within weeks, fatty liver recovers over months or years, and cancer risk falls over years. The table shows how firm each one is. Most come from a single review or study, and they describe groups of people, not you.

Some popular promises have no date behind them. Clearer skin, a day when your energy “returns”, the week your immune system recovers, the exact month your blood pressure settles: none of these has a timetable in the guidelines and reviews above, and calendars that give one are guessing. The NHS does list saving some money among the benefits. Your own bank statement will tell you how much.

What if the first days are too hard to do alone?

Message us before you stop, not after the first bad night. If you drink every day, plan the stop with a medical detox in mind, because stopping suddenly can be very dangerous if you are dependent (NHS). Things can feel worse before they feel better. That is withdrawal, and it is treatable when someone is watching.
